Opt-In/Out Form for Electronic Document Access, Invoicing,  and Voting

This form is for you to
  • select whether you would like to vote online and receive your annual meeting materials electronically (e.g., either online or via email) or to continue to receive hardcopy via US Postal Service and vote via paper ballot.
  • choose to receive your annual dues invoice electronically, hardcopy via US Mail, or a combination of both. 
A reminder that it costs between $3 and $5 in material, labor, and postage for each membership item that is sent hardcopy via US Postal Service.  With over 650 members, this can get quite expensive.  We encourage you to opt-in to electronic means to help keep your membership dues as low as possible.   Items with a red asterisk (*) are required.
