Compliments/Complaint/Comment Form
1. In order for member compliments/complaints or comments to be reviewed by the Board of Directors, a Compliment/Complaint/Comment Form must be filled out, either online below or downloading and filling out the form here and turn into the PVPOA office.
2. The compliment/complaint/comment will be reviewed at the next board meeting.
3. The member will be notified of the decision of the board as soon as possible after the meeting.
4. The complaints/comments are retained on file at the office.
